How Do I Get My Loved One Admitted to Eger?



If your loved one is at home
         
Call a local home care agency and request that a RN come to your home to complete a PRI.  Once completed, the PRI will be faxed by the home care agency to Eger. A staff member of our case Management Department will contact you once the PRI is received to discuss the application process.

 

If your loved one is in the hospital or another nursing home      
Ask the Discharge Planner at the hospital or nursing home to arrange for a PRI to be faxed to Eger( FAX # 718-980-2461).  This PRI be reviewed to determine if the person is eligible for nursing home care. You must complete an application if you are interested in long term care. You may have this mailed or faxed to you.  You may also pick up the application at Eger. You also must arrange for a financial interview. This appointment can be set up by calling extension  718-989-3011 

 

If you are interested in short-term rehabilitation         
Ask the Discharge Planner at the hospital to FAX the PRI to Eger (FAX 718-980-2461). You must provide your insurance information to Eger, as well.

 

If you are in need of Respite Care
This program provides care for a maximum of two weeks at a time up to 42 days per year for those with community Medicaid.   Private pay is also available. 

Call our Social Services Department at ext. 3065 for an application, which includes a medical form to be completed by the community physician prior to the stay at Eger.

 

If Social Day Care would be of interest       
Call our Social service department at ext. 3065.  Flexible hours are available for person with mild to moderate dementia that includes meals and activities in our historic Log Cabin.

If your loved one is at home, in the hospital or in another assisted living facility

Call Eger Harbor House at (718) 554- 8700 for more information and an application. You may have this mailed or faxed to you.  You may also pick up the application at Eger Harbor House. Interested candidates must be 65 years or older with Medicare and Medicaid. Our RN Case Manager will assist you with the pre-admission process. She can be reached by calling
